To establish two specific project areas – one on developing person centred planning processes and the other to focus on improving the outcomes for young people in transition including finding and keeping work. These encompass all the priorities identified at the first meeting of the steering group
-
For all areas of activity, the steering group will work on:
# developing a strong network of key partners and creating partnership links across education, employment, health, housing and social care
# identifying and spreading positive practice and identifying models of effective practice at meetings
# recognising meaningful outcomes that include gaining qualifications and employment
# identifying specific gaps in provision for specific learners and
# identifying where new provision needs to be focussed and developed
# promoting the practice of person centred planning and family engagement
-
Members of the steering group will select the project group that they wish to support and develop.
-
Each project group will report back to the main steering group meeting to update on progress and advise on how best the learning and outputs can be shared and spread across the region
-
LSC members will feed back base line data into the group and inform and direct the project teams to available sources of funding
-
The steering group will direct and support the development of outcome measures from the activities of the project groups that are both quantitative and qualitative and establish critical success factors for the overarching work of the steering group
-
the group will actively disseminate the work of the group to other organisations and individuals with an interest.
